  • Patient and Personnel Safety Policy
  • Incident Report
  • Infection Control Policy
  • Equipment Cleaning Policy
  • Bloodborne Pathogens Policy
  • Acute Medical Emergency Policy
  • Patient Confidentiality Policy
  • Environmental Safety Policy
  • Contrast Administration/Supervision Policy
  • Pre-MR Procedure Screening Form
  • Education, Training and Screening Policy
  • Preliminary Report Policy (if preliminary reports are generated)
  • Quality Control Tests
    • Preventive maintenance (must be within 6 months of submission)
    • Two months daily/periodic QC documentation
    • Latest acceptance test (at installation or after major upgrade)
  • Technical Quality Assessment
    (image quality must be assessed; administrative processes may be included)
  • Interpretive QA Policy
    (peer review and correlation)
